Paid Time Off:
Vacation, sick & holidays all inclusive. All part time employees are eligible. Accrue time from first day employed but unable to start using until after the 3 month initial adjustment period. Part time employee accrues 3.08 hours every 2 weeks during the 1st and 2nd years of service; 3.39 hours during the 3rd through 6th years; 3.85 hours the 7th through 9th years; 4.31 hours for 10th through 14th years; and 4.77 hours 15 years or more. Health Insurance: For all part time employees. Coverage and premiums (pre-taxed) begins the 1st day of the month following 90 days of employment. PPO Plan: WestCare On-site In-Network coverage includes 85%, no deductible. Off-site In-Network includes 80% with $350 individual yearly deductible ($1050 max). $20 co pay (no deductible) for In Network family doctor and $40 co pay (no deductible) for In Network specialist visit. Discount on WestCare hospital bill after insurance pays if you have any type insurance. Supplemental Insurance: Provided by AFLAC. Eligible 1st day of the month following 90 days of employment. Supplemental life, cancer, accident, long-term care, personal recovery, ICU, etc policies. Some premiums are on a pre-tax basis and all are payroll deducted. Dental Insurance: For part time employees. Coverage and premiums (pre-tax) begin the first day of the month following 90 days of employment. $50 individual deductible, $150 family deductible for Type II, Type III and Type IV services. Plan pays 100% (no deductible!) of Usual, Customary & Reasonable (UCR) charges for Type I preventive services. Plan pays 80% of UCR for basic services. 50% of UCR for major services. Plan pays $1500 lifetime per eligible dependent child on orthodontic work (no more than 50%). Plan limit for all other services is $1250 per year per individual. 6 month waiting period for major services (type III). Term Life Insurance: Part time employees eligible to purchase up to 5 times annual salary for self ($150,000 max) and ½ amount chosen for self on spouse ($50,000 max). Part time employees may cover dependent children (up to age 18; 25 if full time student) for various increments if employee purchases supplemental coverage for self. Coverage/premiums (after tax) begin first day of month following 90 days of employment. Premiums based on age and salary. (Optional) Vision Plan: Affordable and convenient eye care...Begins the first day of the month following 90 days of employment. Receive annually one eye exam with a $20 copayment. Once every 12 months receive up to $200 towards cost of eyewear with no office charge (contacts and glasses). Must use participating provider's office. (Optional) Spending Accounts: For part time employees. May begin January 1 of each year. Tax sheltered (pre-tax). Dependent Day Care and Health Care spending accounts. (Pension) Defined Contribution Plan: Employee contribution/employer match 403(B) Pension Plan. Choice of 2 money managers. Employee may begin contributing immediately. WestCare begins graduated schedule match after 1 year employment and completion of 1,000 hours worked for full and part time employees. No match for PRN employees. Current match for employees with 1-4 years of service, get $1 for every $1 you save up to 2%; ...with 5-9 years of service, get $1 for every $1 you save up to 4%; ...with 10-24 years of service, get $1 for every $1 you save up to 5%; and 25+ years of service, get $1 for every $1 you save up to 6%. No match for less than 1 year of service. WestCare match vested: 20% 1st year, 40% 2nd year, 60% 3rd year, 80% 4th year, and 100% 5th year forward. Employee contribution is 100% vested from first contribution forward. Employee Assistance Program: Begins immediately for all employees and dependents. Up to 5 counseling sessions at an EAP Network. Paid by WestCare. Workers' Compensation: Loss of income protection for all employees injured on the job; accidents/job related illnesses. First 7 days employee uses personal sick time, 8th day forward 66 2/3rd % of salary paid by Workers' Compensation insurance. If employee is out more than 21 days, the first 7 days will be reimbursed to employee by carrier. U.S. Savings Bond Program: Employee may immediately begin purchasing U.S. Savings Bonds through payroll deduction. Bereavement Pay: Three days paid by WestCare. Time off for death of a spouse, child, mother, father, step-child, step-mother, step-father, sister, brother, grandparents, parents-in-law, within 30 days of death. Cafeteria Discount: Meals provided to employees at discount and employee may charge via payroll deduction. Parking: Free parking. Gift Shop: Gift shop on Harris campus. May charge purchases via payroll deduction. Jury Duty: Immediate eligibility. WestCare will pay you your regular salary for scheduled time lost if called to serve jury duty. Leaves of Absence: Eligible after 6 months of employment for general LOA's; after 1 year employment and 1,250 hours worked for Family & Medical Leave Act of 1993 (FMLA). Immediate eligibility for military and workers' compensation leaves. For time off for medical, military, educational or personal needs. Service Awards: Quarterly service award luncheons and gift awarded for 5, 10, 15, 20, etc. years of employment. Automatic Payroll Deposit: Direct checking/savings deposits of payroll earnings at most local financial institutions. Waiting period is 1 pay period. ABCD and On the Spot Recognition Programs: Peer recognition and awards for ABCD (Above & Beyond the Call of Duty) and On the Spot Recognition programs. Two separate programs.
